Out of hospital care

ComPacks.

ComPacks - Is a non—clinical package of care management and home care services. It is availabe to patients for up to 6 weeks after leaving an NSW public hospital.

What is provided and how?

Case Management

Patients are allocated to a Case management who will be their main contact throughout their package. The CM assess the patient in hospital, or at home, develop a care plan and co-ordinate some of the following service’s needs:

  • Personal Care

  • Domestic Assistance

  • Transport

  • Meals

  • Social Support

  • In Home respite

ComPacks.

ComPacks -are available for up to 6 weeks. Its additional supports for mob when discharged from a Public Hospital. The ComPacks are 3 Packages.

  1. Safe & Supported @ Home (SACH)

  2. End of life (EoL) Packages/ referrals n patient staff/ Terminal Diagnose (Trauma informed Care)

  3. SACH (NDIS Pathways) & SACH (Aged Care) Packages are made through our referrals and intakes.

  Delivering of Services -

When delivering services, we are professional, our staff are accredited and certified by regulations, we have staff who are qualified and experienced in cultural conform practices & cultural diversity, local staff delivering local services, culturally sensitive and cultural knowledge.

Our Transportation

Our 2 vehicle- Our Bus is a 7-seater with a disability lifter for clients who need additional supports. Our Bus is for Group activities, our new Nissan X-Trail is for out-of-town, travel in style and comfort to Allied Health / Specialist, and also it is use for community supports.

Service Our Way

Service our way is committed to achieving better outcomes for Aboriginal people.

Services Our Way provides culturally appropriate service coordination, support and capacity building for Aboriginal and Torres Strait Islander people and families experiencing vulnerability, empowering them to improve their well-being and achieve their goals.

SOW specialist Aboriginal staff work holistically and collaboratively with clients and service providers. This ensures that a culturally suitable and tailored solution is developed to help address the underlying causes that contribute to daily instability and challenging situations.

At the core of SOW is an individualized and family-centered approach.

The services they offer are -

  • Stable housing Financial management (budgeting)

  • Violence/domestic violence Alcohol or other drugs

  • Mental health Employment relationships / child related

  • General health Family relationships/child related

    Disability Transport.

Sometimes it can be hard when navigating any services, the hub and its staff can assist with filling the EOI form in if needed additional assistance. We understand this can be complex; our services can assist with the paperwork.

Our services.

  • First Nations Social, Emotional and Wellbeing Programs

    This project aims to establish a comprehensive social, emotional and wellbeing (SEWB) program for First Nations people in Inverell. The program will include cultural activities, community outings and self-care initiatives designed to support mental health, enhance cultural connection and improve overall wellbeing.

  • First Nations Advocacy, Referral and Consultancy Services.

    This project aims to establish a comprehensive advocacy, referral and consultancy service for First Nations people in Inverell, operated from the Bandaar Walaaybaa Community Hub. The service will focus on linking First Nations individuals with local services, ensuring their Medicare information is up to date, assisting with MYGOV issues and providing access to various support programs and services.

  • Elder Care Support. Community- led Pathways to care.

    The Elder Care Support program is designed to deliver Aboriginal and Torres Strait Islander Aged Care Support, connection and coordination. Through this program, NACCHO will work with the Aboriginal community- controlled sector to ensure older Aboriginal and TSI people and their families are supported to understand, navigate and access aged care services they are entitles to.
